The Feel Good Suffolk partnership, which comprises Suffolk County Council, Babergh and Mid Suffolk councils, East Suffolk Council, Ipswich Borough Council and West Suffolk Council, is responsible for delivering the Feel Good Suffolk service, supporting people\u2019s health and wellbeing. This means we will collect and use personal and special category data to fulfil these duties.<\/p>\n

This includes processing:<\/p>\n Identifiable data: containing personal data that can identify individuals, such as name, date of birth, gender, address, postcode and NHS number.<\/p>\n Pseudonymised data: this contains information about individuals but with the identifiable details replaced with a unique code.<\/p>\n Anonymised data: this information about individuals has had all identifying details removed.<\/p>\n Aggregated data: this is when all anonymised information has been grouped together so that it doesn\u2019t identify individuals.<\/p>\n <\/p>\n We collect information to deliver the Feel Good Suffolk service in the following ways.<\/p>\n Requests for services via the Feel Good Suffolk (Access Elemental) self-referral form hosted on the Feel Good Suffolk website.<\/p>\n Appointment bookings via Microsoft Bookings \u2013 a link may be shared to enable direct appointment booking with an Advisor.<\/p>\n Requests for services via a GP referral into Feel Good Suffolk (via the System 1 DXS form). Please note, the System 1 DXS Form for Feel Good Suffolk is not used in GP practices based within Waveney.<\/p>\n Requests for information or monitoring support via online forms, such as the Swap to Stop form. Our online forms are built on platforms supplied by a third party called GOSS, a cloud hosted SAAS (Software as a Service). All data is stored, processed and managed in the UK and GOSS are a UK-based service provider. See GOSS Privacy Notice for further information.<\/p>\n We also use Microsoft Forms to collect anonymous feedback data for both the Feel Good Suffolk service and the Feel Good Suffolk website. When you subscribe to the Feel Good Suffolk newsletter, your email address is held on the third party Mailchimp platform to enable us to share email alerts and the newsletter with you. Where applicable this will include referring individuals to Feel Good Suffolk\u2019s third party providers.<\/p>\n Ensuring that people can access support from a provider on a localised basis.<\/p>\n Monitoring and improving the quality of the service we provide based on the timeliness and satisfaction with the service received.<\/p>\n To continuously improve the Feel Good Suffolk service through evaluation of pseudonymised outcome data. This data helps us evaluate the effectiveness of our services and contributes to valuable research in the field of health and wellbeing.<\/p>\n To deliver the Feel Good Suffolk newsletter to individuals who sign up via the newsletter sign up link on www.feelgoodsuffolk.co.uk<\/p>\n To respond to requests for information about the Feel Good Suffolk service.<\/p>\n We will never publish information about individuals and will protect their identities. We will never share information with third parties for marketing purposes, but we will share information with providers, such as leisure centres, stop smoking services etc, who may be able to provide the wellbeing support you need (see below).<\/p>\n <\/p>\n The Feel Good Suffolk Partnership relies on Article 6(1)(a) of the General Data Protection Regulation (GDPR) as the lawful basis on which we use personal data. Any data requests received from a third party will only receive anonymised and aggregated data to a level that complies with the\u202fOffice of National Statistics Disclosure Guidance\u202for, we are required to do so for legal reasons.<\/p>\n <\/p>\n The Feel Good Suffolk Partnership keeps personal data for as long as it is needed to fulfil the purpose that it was collected for, and in line with any statutory or locally determined retention periods.<\/p>\n Can I opt out of datasets?<\/strong><\/p>\n You have the right to opt out from the Feel Good Suffolk Partnership receiving and processing your personal identifiable information.<\/p>\n There are occasions where service providers will have a legal duty to share information, for example for safeguarding or criminal issues.
