You can make a referral by:<\/strong><\/h2>\n

1) SystmOne:<\/strong> Go to Community > Referrals<\/strong> and select Feel Good Suffolk Referral Form<\/strong>.<\/p>\n

2) The Feel Good Suffolk website:<\/strong> Complete the online referral form<\/a>. Select the tick box to indicate that you are referring on behalf of someone else and include your organisation’s details in the text box provided.<\/p>\n

3) Supporting a self-referral:<\/strong> Direct the person to the referral page<\/a> on our website so they can complete the form themselves.<\/p>\n

What happens next?<\/strong><\/h2>\n

All referrals, whether submitted by a healthcare professional or by the individual, are reviewed by a Feel Good Suffolk Advisor. The advisor will contact the individual to discuss their referral, explore their needs in more detail, identify the most appropriate support, and explain the next steps.<\/p>\n

– Have a BMI between 30* and 40 (see below exception)<\/p>\n

*Individuals from minority ethnic groups or with an existing health condition are eligible for support if they have a BMI between 27.5 and 40.<\/p>\n
