Make a referral to Feel Good Suffolk

Get free support to stop smoking, manage your weight or become more active.

 

You can refer yourself using our online form.

Before you start, check the eligibility criteria below to see if we can support you.

Stop Smoking support

You can get support to stop smoking if you:

– Live in Suffolk and are aged 12 and above

– Are a current smoker

Healthy Weight support

You will need to know your Body Mass Index (BMI) – You can find out your BMI on the NHS website. 

You can get support to manage your weight if you: 

– Live in Suffolk and are aged 18 or over 

– Are motivated to achieve a healthy weight

– Have a BMI between 30* and 40 (see below exception)

*Individuals from minority ethnic groups or with an existing health condition are eligible for support if they have a BMI between 27.5 and 40.

Suffolk residents, excluding Waveney:  

If you have a BMI of 40 or above, you should ask your GP for a referral to the local Weight Management and Complex Obesity Service (WMCOS). We cannot accept your referral directly.   

Waveney residents 

If you live in Waveney and have a BMI of 40 or above, we require you to have a consultation with your GP before you submit a self-referral, or they can submit a referral to Feel Good Suffolk on your behalf.  

    Be more active support

    You can get support to become more active if you:

    – live in Suffolk and are aged 18 or over

    – currently do less than 150 minutes of physical activity a week

    – have a BMI below 40

    – can safely take part in physical activity

    Exercise on Referral

    This programme is for people with a stable long-term health condition or disability who would benefit from supported exercise.

    You’ll need a referral from a healthcare professional to access this programme.

    What happens after you make a self-referral?

    Once you’ve submitted your referral, it will be sent to your local Feel Good Suffolk team.

    We’ll review the information you’ve given us and contact you to talk about the support that may suit you
