A self-referral is when a person seeks help or services on their own.
You can make a self-referral to Feel Good Suffolk for support to stop smoking, be active or manage a healthy weight.
Please check the eligibility criteria below before making a referral.

If you do not meet the criteria you may not be eligible to support from a Feel Good Suffolk advisor.
Eligibility criteria for Stop Smoking
You must be:
– Resident in Suffolk
– Current smoker
– Aged 12 and above
Eligibility criteria for Healthy Weight
Firstly, you will need to know your Body Mass Index (BMI) – You can find out your BMI on the NHS website. To access targeted support from Feel Good Suffolk, you must:
- Be a resident in Suffolk
- Be aged 18 years and over
- Be motivated to achieve a healthy weight
- Have a BMI between 30* and 40 (see below exception)
*Individuals from minority ethnic groups or with an existing health condition are eligible for support if they have a BMI between 27.5 and 40.
Suffolk residents, excluding Waveney:
If you have a BMI of 40 or above, you should ask your GP for a referral to the local Weight Management and Complex Obesity Service (WMCOS). We cannot accept your referral directly.
Waveney residents
If you live in Waveney and have a BMI of 40 or above, we require you to have a consultation with your GP before you submit a self-referral, or they can submit a referral to Feel Good Suffolk on your behalf.
Eligibility criteria for Be Active
Please note: we can only provide Exercise on Referral to clients who have been referred by a healthcare professional and meet the criteria to access.
To be eligible for Exercise on Referral, you should be/have:
- Resident in Suffolk, aged 18 years and above
- Inactive (doing less than 150 minutes activity a week as set out by the UK Chief Medical Officers’ guidelines)
- Motivated to be more active
- No contraindications to exercise
- A BMI below 40.
As well as the above you must also have one of the following:
- A stable long-term health condition, e.g, diabetes, cancer
- A disability
To be eligible for wider physical activity provision, you should be/have:
- Resident in Suffolk, aged 18 years and above
- Inactive (doing less than 150 minutes activity a week as set out by the UK Chief Medical Officers’ guidelines)
- Motivated to be more active
- No contraindications to exercise
As well as the above clients/patients must also identify as being from one of the below cohorts:
- People with long-term conditions and limiting illness
- People with disabilities
- People from ethnic minority backgrounds
- People living in deprived areas/on low incomes
- Unpaid carers
- Pregnant women
- People exiting from rehabilitation services
Your BMI must be below 40, (You can find out your BMI on the NHS website)
What support to expect from Feel Good Suffolk
What happens when you make a self-referral?
Your referral will be sent to your local Feel Good Suffolk team. They will review the information you’ve given and contact you to talk about the type of support that may suit you best.
We may contact you by phone, text or email. Please check your junk or spam folder in case our email goes there.
What types of support do we offer?
Feel Good Suffolk is a free healthy lifestyles service for people living in Suffolk. Support is delivered locally, so people can access help that fits their circumstances and the services available in their area.
The service supports adults who want help to:
- Stop smoking
- Manage their weight
- Become more physically active
You can use this website to find information and practical tips to help you get started. For those that meet Feel Good Suffolk’s eligibility criteria and would like more personalised support, a range of targeted interventions are available via a self-referral to a Feel Good Suffolk Advisor. An advisor will contact you to talk through your situation and help you choose the most suitable option.

What do we not offer?
There are a number of support services that Feel Good Suffolk does not offer.
They include:
- Prescription and/or administration of weight loss medication (including injections or tablets)
- Support for those wishing to undergo bariatric surgery

Support to stop vaping (but information is available on our website here: Stop vaping – Feel Good Suffolk).
- Counselling and therapeutic services
- Clinical interventions or advice
- NHS Health Checks
- Child Weight Management
We are also unable to refer clients into clinical support.
Our Weight Management support:
Feel Good Suffolk provides general advice on healthy eating and lifestyle changes. This is suitable for most people looking to lose weight.
If you feel that you need a different type of support, you should discuss your options with your GP. There are other services and programmes which may be more suitable for your needs. These include:
- National programmes for people with specific health conditions such as diabetes or hypertension.
- The local Weight Management and Complex Obesity service (WMCOS). This service offers specialist dietary advice, medications and bariatric surgery, subject to eligibility and clinical prioritisation. More information can be found here: www.suffolkandnortheastessex.icb.nhs.uk/weightmanagement. Your GP will be able to submit a referral to these services if appropriate for your needs.

If you wish to access our Exercise on Referral offer, you need to ask your GP to submit a referral on your behalf. This is so we can confirm that Feel Good Suffolk’s support is suitable and safe for you.
